Put everything a hospital asks for in one printable binder
Fill this in once, on your own time. When something happens, you hand over one PDF instead of digging through drawers and phone notes.
🔒Nothing leaves this device. Your entries are saved only in this browser and the PDF is built on your machine. No uploads, nothing to install. You can export the data as a file if you want a backup.
1 · About them
The basics every intake desk asks for.
2 · Doctors & providers
Primary care first. Add every specialist a hospital might need to call.
3 · Medications
Include over-the-counter stuff and supplements — ERs ask, and interactions matter.
4 · Insurance
One row per policy: medical, dental, drug plan, supplemental.
5 · Emergency contacts
The first row is who gets called first. To reorder, fix the names in place.
6 · Daily routine
If they are admitted or someone else steps in, this is what keeps the day working.
7 · Documents to keep with the binder
Tick what you have. The PDF prints this as a checklist.

FAQ
Is my information uploaded anywhere?
No. The form and the PDF are generated entirely inside your browser tab. Your entries are saved only in this browser on this device, and nothing is sent to any server.
Is the care binder PDF free?
Yes. Unlimited downloads, no watermarks, nothing to install.
What is a caregiver binder?
A single organized document (or folder) with the information you need in an emergency: personal and medical basics, doctors, medication list, insurance, emergency contacts, daily routine and where important papers are. See the full guide on this site.
Can I keep editing the binder later?
Yes. Your entries stay saved in this browser, so you can come back, update a dose or a phone number and download a fresh PDF. You can also export the data as a file to move it to another device.
Does it work on a phone?
Yes, the form and the PDF work on phones and tablets, but a desktop keyboard is faster for the first fill-in.
